Ticket Additional Options

Hello,
How would i add another option to tickets like the priority option but have it display yes or no?
I need a way to mark tickets as significant issues or not. I want to have an option under the priority drop down for this. Currently we are using a custom field but it requires us to edit the ticket itself.
