Inserting / embedding / including signup form on web pages

Is there a plausible way to include just the guestbook signup form to the left column of .html web pages? I would like to have it appear throughout a site, to make signing the guestbook more inviting to visitors, i.e., due to the visibility and the immediacy of having it right on every page.

My desire is that when the form is submitted, the resulting page would be a gbook.php standalone page in a new window, and it would function from there as it does "out of the box." (The signup form on the .html web pages would also have a link to simply view the guestbook, for those who do not want to sign.)

Any suggestions on how to accomplish this? Thanks for any advice.
