I would like to add 4 more comment boxes. So i can give the customer the obtian to add extra information to there tabs. I am using this for a template information form.Can any one Help?

Unfortunately that would take quite some work - out of the scope of my support. If you don't know PHP you might want to hire someone to do it for you.
